You must compete a
Department Checkout Sheet and submit it to the Main Office. Seven signatures are required before you take it to BRWN 2100, and even if you did not use lab
space, it is required that you get a signature (in the"lab safety
manager" line) by Linda Swihart or Paul
Bower.
To get the signature in the lab safety manager line, you must:

Make an appointment for a "work area checkout" from Mr. Bower or Dr. Swihart. Mid-August through mid-May, make this appointment with Linda Swihart by following these instructions (exactly). During the other part of the year or when Dr. Swihart is not available, contact Paul Bower
Do this (request the appointment) well in advance of your departure date, but the appointment should be scheduled near the date you will turn in the last of your keys. |

Then prepare for the checkout appointment in this manner:
- clean your work area completely, desk, hood, sink, floor, shelves.....
- dispose of all chemical "samples" you made or put into a container, as well as purchased chemicals which you received and did not use up. If you are leaving behind ANY chemicals or other bottles or jars which are not manufacturers' packaging (peptides, tissues, organic products, buffers, washes....) you will make a list of what you are leaving behind and the professor will initial beside each row on the list to indicate that you are required to leave them.
- complete the Work Area Checkout Page.

REVIEW -- these steps, for getting ONE of the SEVEN signature lines on the Dept Checkout Sheet filled in (the lab safety mgr line), are all required and cannot be short-cut.
Make the appointment. (Step 1 at left)
Prepare for the appointment by cleaning, disposing, listing....and completing the WAC page/form. (Step 2 at left)
Meet (Bower or Swihart) for the appointment; have three papers ready -- the WAC page/form and samples list to hand over, and the Dept Checkout Sheet for Bower or Swihart to sign in the laboratory safety manager line. (Step 3 at left)

Clean and organize your work area, lab bench,
desk, hood, shelves, cabinets, drawers... so
that they are as nice as you would have liked to have received them when you
got here, or as nice as they were when you did get here, whichever is nicer.
