Checking Out of the Chem Department


These procedures apply to all faculty, A/P, postdoc, grad students, undergrads.... who could be called chemical research or lab staff, and who have used any space in BRWN or WTHR and/or have signed out keys to the building(s) and/or any of the rooms, even if your work area is only office space or a computer room.                            (clerical/service staff go here)

Your Dept. Checkout Sheet must be signed (in the "lab safety manager" blank) by Dr. Swihart.  If you did not use lab space it will be trivial but is still required.

From the Business Office: CHEM Dept Checkout Sheet for faculty, A/P, postdoc, grad students.... 

Email Dr. Swihart to make an appointment.  Do not wait until a few days before you leave to contact her, but the appointment should be scheduled near the date you will turn in the last of your keys.

Start doing it now if you have containers to label and/or waste to dispose.

   

BEFORE the appointment  for the work area inspection, you must prepare and submit information required on the Work Area Checkout page.
(READ AND FOLLOW ALL directions carefully.)


     1. Print the Work Area Checkout page.
    2. Fill it out.  (It is not optional)
    3. Print & complete samples if necessary

Have these ready at the appointment with Dr. Swihart.

Complete and send > 24h before appointment time.


Clean and organize your work area, lab bench, desk, hood, shelves, cabinets, drawers... so that they are as nice as you would have liked to have received them when you got here, or as nice as they were when you did get here, whichever is nicer.
