Intro: Combined BRWN/WTHR inspection report.  Supervisors' names ("responsible" column) added via info from Space data table, email corrections or questions.

Correction of items must be reported to the Chemistry Safety Committee.  To do this:
    1. print the relevant portion(s) of the table,
    2. mark "CORRECTED" and a date by each corrected item
    3. get a signature beside each corrected item from the person whose name is in the
"responsible" field for the space , and
    4. give to the Safety Secretary (Susan Greer) in BRWN 4103 (Organic Office)

"See Notes" pages

Report supervisor-room number mistakes to Linda Swihart
Request clarification of report items from Judah Young

As of 03/22/2005 12:54 PM, there are still a few corrections not reported.  Check your records below; a date in the "date corrected" field is the important indicator; the dark gray background is for ease in finding the unfinished items.

Physical Facilities will give us our re-indemnification letter after the report violations are taken care of.  (The 30-day and less ones done, the others formally promised.)

Note on meanings of recommended abatement period and indemnification requirements:

Interpret "Immediate" to mean do it as soon as is humanly possible.  Then notify as per instructions above with date and signature beside each item corrected.  Note that Chemistry will not be re-indemnified until we have
(a)
fixed all the 30d and less-than-30d items, and
(b)
formally committed to the longer term ones.

What we do right.... is not noted on the inspection form.  Too damn bad, but it's just a whole lot more than we pay enough people to do.  They (REM people, everybody) know we do more stuff right than wrong.  They remark on it during inspections when a chance arises and they think of it.  But the inspections are not about congratulating your lab for the things it does right.  They are for focusing on the deficiencies.  Don't let that give you the impression that the deficiencies predominate; they don't.

Physical Facilities items:   These below will be taken care of by Physical Facilities (Sign Shop and other sections of Facilities Services, Fire Safety & Equipment Services....) and REM will be notified by those sections; no action is required from other staff.  However, if you happen to notice that something here has still not been corrected after some length of time, please feel free to do us the favor of mentioning it in an email to me.
