An individual's safety training is the legal responsibility of the individual's department and the individual's supervisor  --  the professor in the case of all who work for faculty.  This responsibility is not negotiable and cannot be reassigned.   Introductory Lab Safety Training & Review is offered by REM to help Departments and supervisors fulfill some of their training responsibilities.

Each person complete his/her own form, i.e. nobody signs up another person.  The responses provided by the prospective attendee give valuable information to the training meeting instructor which allow some tailoring of the content of the session.  THIS WILL TAKE 10 - 20 minutes to do correctly (after you've done the CHP1 and PPE online parts):  Follow all instructions, complete all of the information requested, and then click on the SUBMIT FORM button at the bottom and print the confirmation page.  NOTE THAT YOU MUST know and communicate quite a bit about the work you will be doing.  Take the time to provide complete answers.
