Have you been informed of:
the contents of the OSHA lab standard and its appendices and where you can see a copy of them?

Yes.  The training module in its entirety does just that, including informing you that a copy of the lab standard may be obtained from the OSHA website Lab Standard Section.

the location and availability of the Purdue CHP.

Yes.  The Purdue Chemical Hygiene Plan is available from REM at its web site, go to REM CHP Manual Links to choose html or PDF, and can also be obtained as a printed document.

the location and availability of safety information

Yes.  Safety information available in the Science and Engineering Libraries (Chemistry has the most), as well as from each Department's safety committee, from the internet (use the links on the REM web page at http://www.purdue.edu/rem/ih/msds.htm), as well as from your supervisor, and from container labels.

the signs and symptoms of exposures to the hazardous chemicals used in your workplace.

Maybe.  This online training does not cover the specific signs and symptoms of exposure to every chemical that exists and therefore might not include everything you will find in your work area.  It does broadly cover information on chemical exposure signs and symptoms in general.  If you believe that the coverage there has included the chemicals with which you will be working, it is appropriate to answer this question YES.  If you need more information on specific chemicals, do not hesitate to contact REM or your supervisor.

the Permissible Exposure Limits for the hazardous chemicals used in your workplace

It's another maybe.  The online information explains PELs, gives examples, and points you to where you can find the OSHA-established PEL lists.  This may have covered the topic thoroughly enough to warrant your YES answer.  If it has not, again, contact REM or your supervisor for more information.

Have you received training about:

These four items above are likely to be only partially addressed by this online reading assignment, because this is a very basic level introduction, not meant to cover every chemical, every method of detection, or every kind of workplace.  However,  Federal law requires that your answers be yes or your employer has not done its duty.  (It's not you who's in trouble if you are incompletely trained or inadequately informed.  It is your employer, in the form of your supervisor.)

Before you answer yes to these items you must believe that you have received adequate and appropriate training about those topics.  Until you have, you must say no and you must not undertake work with hazardous chemicals or other hazardous operations.

Contact your supervisor, or your Department's safety committee, or REM.  The REM main office number is 46371 and say that you need to speak with someone about LAB SAFETY TRAINING, and you will be given the phone or email of the appropriate person.
