Register for "Basic Lab Safety Training & Review," aka "CHP2."

The instructions about who has to do what kinds of training are elsewhere

An individual's safety training is the legal responsibility of the individual's department and the individual's supervisor  --  the professor in the case of grad students and other research staff.  This responsibility is not negotiable and cannot be reassigned.   Introductory Lab Safety Training for Lab Staff is offered by REM to help Departments and supervisors fulfill some of their training responsibilities.

(1) online independent study in TWO parts;
    (a) "CHP1" -- a fairly in-depth overview of the Chemical Hygiene Plan, and
    (b) "PPE" Personal Protective Equipment training,
(If it has been more than 3 months since you completed the online exercises, or you have forgotten and not kept a record, do them again.

(2) Sign up for (and attend) the 110-minute small group training session (called "CHP2") which covers, for each worker, the very work area specific hazards of his/her job.  (Sign-up form continues below.)

Print and bring with you to the training session the confirmation forms from 1(a), 1(b), and 2.  The instructor cannot admit you without them.

Note that the sessions are 110 minutes, and usually fit between scheduled class times.  If you cannot fit any of the shown times into your schedule, please follow the instructions for completing the ScheduleCard below.  Groups of 5 - 10 are ideal, but "groups of one" are not uncommon.

Provide Information & Sign-Up for 110-min "CHP2 Training" meeting.
You must be able to print the confirmation form after submitting this web form.
Each person is to complete his/her own form, i.e. nobody signs up another person.  The responses provided by the prospective attendee give valuable information to the training meeting instructor which allow some tailoring of the content of the session.  THIS WILL TAKE 10 - 20 minutes to do correctly (after you've done the CHP1 and PPE online parts):  Follow all instructions, complete all of the information requested, and then click on the SUBMIT FORM button at the bottom and print the confirmation page.  NOTE THAT YOU MUST know and communicate quite a bit about the work you will be doing.  Take the time to provide complete answers.

1. Enter date (mm/dd/yy) on which you last completed the CHP1 online reading exercise:

(Do not enter a date in the future.)

The instructions above include what to do if you've forgotten these dates....

2. Enter date (mm/dd/yy) on which you  last completed the PPE online reading exercise:
or enter NO PPE NEEDED.  (Do not enter a date in the future.)

3. Sign up for:
(the earliest
time you
are able)

Do not sign up for a session you will not attend.

There are no other sessions offered by REM during summer 2008.  Any other training needs must be met by your department.  Note that all staff and student researchers are required by law to receive adequate training and information before they work with any hazards.  There is no requirement that it all come from REM.
