FILL IT OUT ON-SCREEN by entering your data into the fields (yellow background), and then print it. You will not be able to save your filled-out version unless you have Acrobat Exchange (the Acrobat Reader is freeware, but Exchange is the non-freeware program from Adobe which provides for editing of pdf files), so be sure to print your form(s) before they are lost from the browser's cache.
ALTERNATIVE - print blank copies of the shipping request form and complete
them by hand or type them.
